Patients living within the practice area are welcome to register and you can confirm your eligibility from your postcode, simply click on the tab Practice Boundary and put in your postcode. If you live out of the practice boundary and are choosing to register at our practice, your registration may need approval first and reason for wishing to register. the Surgery does not provide home visiting to out of boundary patients listed, you will be signposted to another emergency service and your area of residence may affect other services that you require or become needing to require for your health and well-being! 

We ask that you do bring a form of ID with you in order that we establish correct spellings for registration.

You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. You are advised to ensure that until full registration is complete that you have enough of any medications you are taking from your previous practice as we will not be able to prescribe until you have been seen by one of our GPs.

It is important that ethnicity is given so that your records are detailed in regards to 'risk' factors due to the COVID pandemic.  We also ask that you willingly attend a new patient registration medical check up.
